How to complete the Membership Form
Choose whether to print the membership form off and send in by post, or whether to download to your computer and send in by email.

Complete and send by email
Download this Ms Word document. Firefox and Explorer users should right click on this link and choose 'save link as..' or 'save target as' to save it to your computer. Then open up the Ms Word document and complete the fields required (you can use an 'X' instead of entering a 'tick'). Then save the document. Email the saved document to nas@mugsy.org.

Complete and send by post
Download this PDF file to your computer. Then print off and complete the form. Send by post to Nancy Rowell, 161 Sycamore Road, Farnborough, Hants. GU14 6RF. N.B. This version of the membership form cannot be completed on your computer. This version of the membership form is in PDF format and requires the Adobe Acrobat Reader installed on your Windows PC.

Please note that the above form is just for membership of the NAS Surrey Branch, not membership of the National Autistic Society as a whole. There is no charge for Branch membership, but we would encourage Branch members to also join the National organisation. The Branch receives a proportion of the subscription paid by National members who nominate us as their local branch, which helps with our running costs, including the cost of producing our newsletter.
